Request for a Withdrawal from the University

Student Instructions

Definition: A withdrawal implies permanent departure from the University without the immediate expectation to return.

Making the decision to withdraw from the University is an important one. Familiarize yourself with the University Leave of Absence and Withdrawal Policy (including the tuition refund policy, Title IV refund policy, and housing termination policy).

Required Steps:
Below are the steps you will need to take in order to a withdrawal from your academic program.

  1. Complete the Withdrawal Request form
    A letter which details the circumstances for your request must accompany this form.

     
  2. Meet with Your Academic Department
    If you are considering withdrawing from your academic program, you should meet with your academic department as soon as possible. Your academic department will counsel you and guide you throughout the withdrawal process. You should initiate this process well in advance, ideally at the end or beginning of a quarter.

     
  3. Meet with Student Service Departments
    Your academic advisor will require that you complete a Student Clearance form before a withdrawal from the University will be approved. This is a vital step to ensuring that you fully understand the impact your change in student status will have on your financial aid, student billing account, housing, health insurance, and other issues.

    Financial Aid Office: All student loan borrowers are required to complete a financial aid exit interview with the Financial Aid Office. 

    Student Housing: If you are a resident of student housing, you will need to meet with the housing office to discuss your plans and your housing contract.

    Registrar’s Office: Meet with the Registrar’s Office to withdraw from courses and update your address.

    Business Office: Discuss your final billing.

     
  4. Receive final approval from Academic Department
    Your request to withdraw from the University is not approved without the signature of your academic department.

     
  5. Relinquish Your Student ID Badge and University Keys
    You must return ID badges and university keys to your academic department at the time of final approval. If you are an on-campus resident, you will turn in your ID badge, residential mailbox key, and residential door keys as directed by Student Housing.
