2012 – 2013 INSURANCE PLAN YEAR BEGINS 7/1/2012. 
New contracts for health, dental and vision insurances should be finalized by 6/2012. 
At that time, the 2012-2013 insurance rates will be updated below.
All students currently covered by RFUMS-provided insurance, will be mailed
NEW 2012-2013 insurance ID cards which will replace the 2011-2012 ID cards
There is no need for currently covered students to re-apply, nor students who have
previously waived to waive again if they continue to have their own insurance coverage. 
Exception:  Students who have graduated from one program and will begin another in the
fall term must re-apply for insurance, or submit a waiver with proof of private insurance. 


Health Insurance 

All full-time students are required to obtain and maintain health insurance while enrolled at RFUMS. In order to ensure that all students are in compliance with this requirement, the University offers group health insurance provided by Maksin Management Corp. Dependent coverage may also be purchased from Maksin. 

You must submit either a Health Insurance Enrollment Form or a Health Insurance Waiver Form by the 10th business day of your first academic term enrolled at the University. The waiver must be accompanied by proof of current coverage from another insurance company.   If you do not submit either a Health Insurance Enrollment form or a Health Insurance Waiver form, you will automatically be enrolled in the Maksin default plan (Plan B). 


Dental and Vision Insurance 

The University offers group dental and vision plans provided by Guardian. Students may elect to purchase either or both of these plans at initial enrollment or during annual open enrollment periods (June 1st thru June 30th each year). Please note that if students elect to enroll in the dental plan they will not be allowed to cancel their coverage for a 1 year period of time and the vision plan requires a 2 year enrollment. Cancellation of coverage must also be done during the open enrollment periods (June 1st thru June 30th each year).

The only exception to these policies, which are required by the insurance company, is when a student leaves the University the coverage terminates at the end of the month in which the student leaves. Following are links to the plans that describe benefits as well as other informational documents.
