At times, the step of disclosing certain interests needs to be formalized in a written disclosure report. Further information about that requirement is contained in the policy "Written Disclosure Reports." Please note that it is possible for one individual to be required to forward a copy of the disclosure report to more than one reviewer, due to the various roles held by that individual.
