Application Checklist

The application checklist is designed to guide you through the submission of all application materials. An application is not considered complete, and will not be reviewed by the Physical Therapy Program Admissions Committee, until all PTCAS documents, GRE scores, and Supplemental fee are received by RFUMS. Once your application is considered complete, you will receive email notification. 

  • Complete PTCAS Application
    The application is available beginning July 1, 2010. It is of the applicant's best interest to submit all materials as early as possible. All completed applications received by October 15, 2010 will receive full consideration. Review of applications received after October 15th will continue until December 1, 2010 or until the class is full. 

    Remember all letters of recommendation and transcripts are sent directly to PTCAS.
    If you have difficulty with the PTCAS site, you may call 617-612-2040 to report problems.   

     
  • Graduate Record Examination
    Completion of the GRE is required. Rosalind Franklin University’s ETS site code is 1117, and the department code is 0619.

 

  • Submit Supplemental Fee to Rosalind Franklin
    A $30 processing fee is required. At this time, credit cards are not accepted for payment. Please place "DPT Admissions" in the memo line and send check to:
                         Rosalind Franklin Unviersity of Medicine and Science
                         Office of CHP Admissions
                         3333 Green Bay Road
                         North Chicago, IL 60064-3095
