In June 2011, Rosalind Franklin University Health System took ownership of the CareCoach, a mobile health screening unit, from Northwestern Lake Forest Hospital. We’ve given the vehicle a new look and a new name – the Community Care Connection – and we’re visiting communities throughout Lake County to provide free health and wellness screenings!

About the Community Care Connection

The Community Care Connection is staffed by a dedicated team of healthcare professionals offering a variety of health screening services – free of charge! The mobile health unit also provides invaluable learning experiences for students from Rosalind Franklin University of Medicine and Science, who will assist with the screenings under the supervision of licensed healthcare providers.

Individuals that visit the Community Care Connection for screenings receive health counseling and educational materials based on their screening results. Referrals are provided to patients who need follow-up care.

Free Health Screenings Include:

  • Blood Pressure
  • Blood Sugar/Diabetes
  • Cholesterol
  • BMI (body mass index)
  • Waist Measurements
  • Foot Screenings*
  • Osteoporosis*
* These screenings may not be available at every location.
